- Core responsibilities: conduct comprehensive PT evaluations, design and implement individualized treatment plans, apply evidence-based interventions to restore function and reduce impairment, monitor patient progress, adjust plans as needed, provide patient and caregiver education, document outcomes accurately, collaborate with physicians, nurses, and therapists, and participate in interdisciplinary rounds to optimize care. You’ll also engage in risk assessment, mobility training, gait retraining, tailored gait and balance programs, and discharge planning that supports long-term independence.
