Role specifics and benefits at a glance: Your primary responsibilities center on delivering hands-on nursing care with an emphasis on safety, comfort, and individualized care. Administer prescribed medications accurately and on schedule, monitor and document vital signs, manage wound and skin care, assist with activities of daily living, and ensure fall prevention and mobility support. Conduct ongoing assessments to detect changes in condition, initiate timely interventions, and communicate updates to the supervising RN, medical directors, and therapy teams. Contribute to the development and refinement of comprehensive, resident-centered care plans, aligning goals with family wishes, regulatory standards, and best practices in geriatric care. Maintain meticulous documentation in the electronic health record, perform routine chart reviews, participate in interdisciplinary rounds, and support continuity of care during transitions between services or facilities. Engage in patient and family education on disease management, medication use, safety strategies, and wellness activities, fostering trust and partnership with residents and caregivers. Remain alert to infection prevention protocols, respiratory etiquette, wound care standards, and pain management, adapting approaches as needs evolve.
