Role specifics and benefits:
- Core responsibilities: deliver holistic long-term care as an LPN, including vital sign monitoring, medication administration and reconciliation, wound care, ADL assistance, tube feeding and catheter care as appropriate, basic IV management where permitted, and timely documentation in electronic health records. Collaborate with RNs, physicians, social workers, and therapy teams to implement individualized care plans, recognize and report changes in patient condition, identify safety risks, prevent falls, and support infection control and quality improvement initiatives. Provide patient and family education on diagnoses, medications, and care routines, and assist with discharge planning and transition support when needed.
