Core responsibilities include comprehensive in-home assessments, wound and IV care within established care plans, medication administration and reconciliation, training family caregivers, and coordinating with physicians, social workers, and vendors to ensure seamless transitions of care. You’ll monitor patient progress, adjust plans as needed, document outcomes, and uphold the highest standards of infection control and safety. This role offers clear pathways for professional growth in Home Health, including expanding senior-focused competencies, palliative and hospice integration, and leadership opportunities within travel teams or regional programs. You’ll receive a competitive compensation package with a weekly pay range of $1,670 to $1,787, reflecting your experience and the travel demands, plus a guaranteed 40 hours per week. A welcome or retention bonus may be included, and housing assistance is provided to ease relocation and travel living. We also offer contract extensions that let you deepen your engagement with patients and teams, building continuity of care for families.
