Role Specifics and Benefits:
- Key responsibilities: provide comprehensive nursing care to residents in long‑term settings, including initial and ongoing assessments, care planning, medication administration and reconciliation, and execution of individualized treatment regimens; implement fall prevention, infection control, pressure injury prevention, wound care, and care for chronic conditions; monitor vitals, respond to emergencies, and document changes in condition promptly; assist with activities of daily living, mobility training, and therapy plan coordination; collaborate with physicians, social workers, therapists, families, and facility staff to align on goals and discharge planning when appropriate; maintain meticulous records and ensure regulatory compliance, quality metrics, and safety standards are met; mentor and support CNAs and other teammates to elevate daily care and patient satisfaction; participate in quality improvement initiatives and stay current with geriatrics best practices and emerging evidence.
