In this RN LTC role, you’ll deliver comprehensive, patient‑centered care tailored to the unique needs of residents in long‑term care settings. Core responsibilities include thorough initial and ongoing assessments, development and adjustment of individualized care plans, medication administration and monitoring, IV therapy when indicated, wound care, and vigilant safety protocols. You’ll monitor vital signs, recognize subtle changes in health status, coordinate seamlessly with physicians, therapists, social workers, and families, and document meticulously in electronic health records to ensure continuity and quality of care. Education for residents and families on disease management, treatment options, nutrition, fall prevention, and medication adherence will be a daily emphasis. You’ll mentor junior staff, participate in care rounds, lead by example in infection control and pain management, and seize opportunities for professional growth within the LTC specialty.
