In this Long-Term Care LPN position, you will perform essential clinical duties with compassion and precision. Your responsibilities will include thorough patient observations, administration of medications and treatments according to physician orders, monitoring vital signs, and contributing to the development and execution of individualized care plans. You will assist residents with activities of daily living, support mobility and rehabilitation goals, participate in wound care and disease management, and document care with accuracy to ensure seamless communication among the care team. Your role will also involve collaborating closely with registered nurses, charge nurses, therapists, and family members to anticipate needs, address concerns, and respond to changes in residents’ conditions.
