In this RN Case Management role, you’ll be at the core of patient success. Key responsibilities include thorough clinical assessments and risk stratification, developing individualized care plans, coordinating services across multidisciplinary teams, arranging safe and timely discharges, and ensuring smooth transitions across inpatient, outpatient, and community settings.
