As a Travel RN in LTC, you’ll be at the heart of resident wellness, collaborating with interdisciplinary teams to deliver comprehensive, individualized care that respects dignity and independence.
- Key Responsibilities:
• Conduct thorough assessments of residents’ physical, cognitive, and emotional health, adjusting plans as needs evolve.
• Safely administer medications, manage IV therapies, and monitor for adverse effects, with meticulous documentation in the EMR.
• Develop, implement, and update individualized care plans in coordination with families, physicians, social workers, and nursing assistants.
• Provide wound care, pressure injury prevention, mobility support, and falls risk mitigation to maintain optimal functional status.
• Support activities of daily living, promote nutrition and hydration, and assist with rehabilitation goals as appropriate.
• Monitor chronic conditions, perform routine screenings, and recognize early indicators of deterioration requiring escalation.
• Facilitate smooth transitions of care, discharge planning, and communication with external providers and family members.
• Uphold strict infection control practices, safety protocols, and compliance with regulatory standards.
• Educate residents and families about treatment plans, medications, and self-management strategies to empower informed decisions.
