You’ll develop and implement individualized wound-care plans, monitor progress with standardized metrics, document outcomes accurately, and coordinate with physicians, case managers, and therapists to ensure seamless transitions from hospital to home or outpatient settings. You will educate patients and families about wound care, dressing changes, nutrition, edema management, and activity modification to promote healing and prevent setbacks. The role offers clear pathways for professional growth: pursue wound-care certifications, engage in quality-improvement initiatives, and participate in interdisciplinary rounds to influence evidence-based practices.
