- Core responsibilities: perform comprehensive assessments, monitor resident conditions, administer medications and treatments per individualized care plans, document precisely and timely, recognize early warning signs of deterioration, and communicate changes to RNs, physicians, therapists, and family members. Provide wound care, skin integrity management, and supportive therapies as directed; implement and reinforce infection control practices; educate residents and families on disease processes, medications, and routines; participate in multidisciplinary rounds and care conferences; ensure safe transfers and continuity of care between shifts and settings.
