In this Long‑Term Care LPN role, you will: provide direct patient care, including medications administration per physician orders and nursing protocols, routine assessments, wound care, skin integrity management, and monitoring of chronic conditions; perform vital signs checks, identify trends, and communicate changes promptly to the supervising RN and physician teams; assist residents with activities of daily living, mobility support, personal hygiene, nutrition, and fall prevention; document thoroughly in the EMR and contribute to accurate care planning, progress notes, and shift handoffs; support visitors and families with clear explanations of care plans, prognosis, and available resources; supervise and mentor CNAs on your shift, ensuring safe, compassionate care and adherence to infection control standards; participate in care rounds, discharge planning, and transitions to higher levels of care when necessary.
