As a Long-Term Care LPN, you will be a hands-on cornerstone of resident care. Key responsibilities include accurately administering medications and treatments under supervision, monitoring vital signs, assessing condition changes, assisting with activities of daily living, wound care and dressing changes, and delivering timely documentation that informs care plans. You will support registered nurses and care teams in implementing individualized plans, communicating care goals to residents and families, and ensuring safe transitions between units and locations.
