Role Specifics and Benefits
Responsibilities:
- Provide safe, patient-centered nursing care to adults and older adults in long-term care, including monitoring vitals, assessing needs, and delivering skilled interventions under supervision.
- Administer medications, manage IVs and wound care, perform basic procedures, and ensure safety through best-practice protocols and infection control.
- Assist with activities of daily living, mobility, and rehabilitation goals; support nutrition, hydration, and comfort measures.
- Develop, implement, and monitor individualized care plans in collaboration with RNs, physicians, therapists, and families; document progress accurately in electronic medical records.
- Recognize changing conditions, respond promptly to alarms, and coordinate with interdisciplinary teams to adjust plans of care.
- Provide clear, compassionate communication with residents and families; advocate for residents’ preferences and rights; support end-of-life care discussions when appropriate.
- Participate in quality improvement initiatives, safety rounds, and ongoing education to elevate the standard of LTC practice.
