Role Specifics and Benefits: In this LPN Long-Term Care role, you’ll deliver essential nursing care under the supervision of a registered nurse, focusing on supportive, respectful, and anticipatory care for residents with chronic conditions, dementia, and post-acute needs. Key responsibilities include accurate medication administration, routine head-to-toe assessments, timely documentation, and updating care plans in collaboration with the care team. You’ll assist with activities of daily living, monitor pain levels, manage wound care and dressing changes, support mobility and fall prevention, provide skin integrity oversight, and communicate observations to families with compassion and clarity.
