In this position, you’ll perform key responsibilities that keep outpatient clinics efficient and patient-centered. Core duties include comprehensive patient assessments, documenting and prioritizing needs, monitoring vitals, administering medications as directed, assisting with minor procedures, wound care, and changing dressings when appropriate. You’ll conduct patient education on treatment plans, medication management, preventive care, and post-visit instructions, strengthening adherence and outcomes. Coordinating referrals and follow-ups, communicating with multidisciplinary teams, and supporting discharge planning are essential. You’ll track patient outcomes, contribute to quality improvement initiatives, and participate in the ongoing development of evidence-based care pathways.
