As a Hospice Registered Nurse, you’ll assume primary responsibilities for clinical assessments, symptom management, and the creation and execution of individualized care plans. You’ll perform comprehensive patient assessments, monitor evolving conditions, administer medications and non-pharmacologic interventions, and adjust plans as needs change. You’ll coordinate with physicians, social workers, chaplains, nurses’ aides, and home health teams to ensure seamless care transitions, arrange equipment, and advocate for patient goals and comfort. Education for patients and families on disease trajectories, treatment options, and end-of-life comfort measures will be essential, ensuring informed choices and compassionate support. You’ll document meticulously in the electronic health record to support continuity of care, regulatory compliance, and quality outcomes. Participation in ongoing quality improvement initiatives and safety programs will help you refine practice and contribute to a culture of excellence.
