- Core responsibilities: Deliver comprehensive home-based nursing care to diverse adult and elderly patients, including initial and ongoing assessments, development and adjustment of individualized care plans, wound care and dressing changes, IV therapy and infusion support, insulin administration and diabetes education, pain and symptom management, medication reconciliation and adherence support, and patient and caregiver education on disease management, safety, and preventive health. Coordinate with physicians, therapists, social workers, and other clinicians to ensure seamless care transitions, while documenting accurately in electronic health records, maintaining HIPAA compliance, and adhering to agency policies and regulatory standards.
