As an LPN in Long-Term Care, your responsibilities include direct, compassionate patient care: taking and monitoring vital signs, administering medications and treatments within scope of practice, supporting activities of daily living, assisting with mobility and rehabilitation, wound and skincare, catheter and ostomy care as appropriate, and ensuring timely documentation in electronic medical records. You will collaborate closely with registered nurses, physicians, pharmacists, social workers, and families to update care plans, manage pain, prevent falls, and promote safety and infection control. You’ll participate in care conferences, perform routine assessments, and respond to changes in residents’ conditions with calm, professional judgment.
