Key responsibilities center on evidence‑based assessment, goal‑driven treatment planning, and outcome‑driven progression. You’ll conduct comprehensive evaluations, develop PT plans of care, implement therapeutic exercise programs, apply manual therapy techniques, and monitor progress toward functional goals. You’ll lead gait and balance training, assistive technology applications, and functional mobility coaching, ensuring safe transitions to home or community environments. Documentation in electronic health records is meticulous, timely, and compliant with payer guidelines. You’ll collaborate with nurses, physicians, case managers, and therapy aides to coordinate care, participate in case conferences, and contribute to discharge planning and post‑discharge follow‑up when applicable.
